MacBook Loss/Theft/Damage Procedure
As outlined in the Laptop Computer Use Agreement, employees are responsible for partial replacement cost of laptops in the event of loss, theft or damage.
Below are procedures for the process:
The employee shall:
- Notify the campus principal within 72 hours
- File a police report within 72 hours
- Provide a copy of the police report to the campus principal, director of Instructional Technology and director of Risk Management
- Pay a deductible of $750
- May be done through payroll deduct, if desired (more information below)
- If the employee would like to write a personal check for the full amount, make it payable to "Ysleta ISD" and forward to the campus principal.
The principal:
- Will collect a check from the employee for the $750 deductible in full and forward to the YISD Cashier (in Accounting) with information on the account number to be credited (6645), OR
If using payroll deduct:
- Will write a memorandum to Kathleen Taylor (in Payroll), which includes:
- The employee's full, given name
- Social security number
- Full amount due ($750) and whether:
- Payment will be made in full through payroll deduct, or
- Payments will be made in installments through payroll deduct (Contact Kathleen Taylor at extension 40271 for more information on installments)
- The complete campus account number to credit the account (6645)
- A copy of the signed Laptop Computer Use Agreement
- A copy of the police report attached to a completed transfer form (to delete the laptop from inventory).
Once the above is complete, the campus:
- Will order a comparable replacement laptop unit and pay the difference in cost with campus funds.
- Will enter the replacement unit into inventory and note that this is a replacement unit.
